Open Enrollment is July 1-20! Now is the time to apply for healthcare coverage for yourself and your children, or make changes to existing coverage.
What You Get
For just $25 per month, you get the following benefits:
Additional Benefits Now Available
Starting August 1, healthcare coverage for caregivers will also include:
- $0 co-pay for preferred brand or preferred generic insulin.
- Increased alternative care (such as acupuncture and chiropractic care) covered for up to 20 visits per year.
- Unlimited naturopathic care.
- An improved infertility benefit, including a new pharmacy benefit for fertility needs.
Health Plan Information
You have access to affordable, high-quality healthcare coverage for caregivers. Your specific health plan is automatically assigned based on your ZIP code. You can choose from 2 dental plans: Delta Dental or Willamette Dental.

About Open Enrollment
Open Enrollment is your yearly chance* to apply for healthcare coverage, get Coverage for Kids and/or make optional changes to your coverage (if you are enrolled).
Eligibility:
- If you are eligible now and apply for coverage, your coverage will start August 1.
- If you are not eligible right now but are interested in coverage, you can still submit a completed Health Benefits Application by July 20. Your coverage will start when you become eligible.
- If you are already enrolled, you do not need to take action unless you want to get Coverage for Kids or make changes to your dental plan.
Coverage for You
You are eligible for personal healthcare coverage when you work 80 or more per month 2 months in a row. Log into Health: My Plan to view your eligibility information.
Coverage for Kids
If you work 120 hours or more per month, you can now get the same high-quality medical and dental coverage available to you, for your kids (through their 26th birthday)! You can choose from two options:
- Medical and dental Coverage for Kids for $100 a month, plus $25 for your coverage.
- Dental-only Coverage for Kids for just $10 a month, plus $25 for your coverage.
If you choose to get Coverage for Kids, you will need to submit a copy of a document that verifies your relationship to your dependent children (Dependent Verification). Your children’s coverage start date will vary depending on when your Dependent Verification is received.

How to Enroll or Make Changes
You have 2 options to apply or make changes to your coverage during Open Enrollment. Your application must be received by July 20 for it to be processed.

You were sent a Health Benefits Guide and a Health Benefits Application in the mail to fill out and submit (sent in June). Postage is required. Mail it in an envelope with your return address to:

*A Qualifying Life Event is a change in your life situation – like losing other coverage or having a baby – that can make you eligible for a 30-day Special Enrollment period, allowing you to enroll in health coverage outside the yearly Open Enrollment. The 30-day period begins at the time of your event and you must complete and submit your application within the 30-day time frame. For a full list of Qualifying Life Events, visit Get Covered.
