If you work 120 hours or more per month, you can now get health coverage for your kids through their 26th birthday!

With Coverage for Kids, your kids can get medical and dental coverage for $100 a month or dental-only coverage for just $10 a month. Your kids get all the same great benefits you do, including emotional well being, orthodontia, wellness visits and much more.

How much does it cost?

If you apply for Coverage for Kids, you will need to choose between two coverage options:

  • Medical and dental Coverage for Kids for $100 a month (plus $25 for your coverage).
  • Dental-only Coverage for Kids for $10 a month (plus $25 for your coverage).

The monthly payment for Coverage for Kids is the same amount, no matter how many kids you have!

How do I get Coverage for Kids?

To get Coverage for Kids, you must work 120 hours or more a month and continue to work 120 or more hours a month to keep coverage. If you are not currently working 120 hours or more but would like to get Coverage for Kids, you can still submit a completed Health Benefits Application during Open Enrollment. Coverage would start when you start working 120 hours or more a month.

How do I add a Child?

There are several steps to enrolling your children on your coverage and verifying your relationship to them.

The easiest way to apply for Coverage for Kids! 
By Mail
  • Fill out the Dependent section of the Health Benefits Application, and submit it using Health: My Plan.
  • Submit a document that verifies your relationship to your dependent(s) within 60 days of submitting your Application using Health: My Plan.
  • You will get a confirmation letter within 30 days, by mail or email.
  • Fill out the Dependent section section of the Health Benefits Application, and send it to the address on the Health Benefits Application by July 20.
  • Include a copy of a Dependent Verification Document for each child.
  • Begin paying through payroll deduction.

Dependent Verification

When you choose to add children to your coverage, you will need to submit a Dependent Verification document that verifies your relationship. A document is required for each child you add.

More Details About Dependent Verification:

  • It is recommended that you submit your Dependent Verification document(s) with your application, but you have 60 days after your application is received to submit it. Your application is not complete until you send your Dependent Verification. 
    • If you send your Dependent Verification separately, please make sure to label it with your first and last names and the last four digits of your Social Security number.
  • Make sure to send a copy and not the original document.

Who Can I Add to My Coverage?

You can add eligible children (through their 26th birthday) in Coverage for Kids. Below is a list of qualified children . Click the “+” to see which documents you can use as verification for each type of dependent.

You can enroll your biological child (also known as your child by birth) in Coverage for Kids. Here is a list of acceptable documents:

  • Government-issued birth certificate that includes your name as a parent.
  • The most recent year’s federal tax return that includes the child(ren) as a dependent(s) and listed as your child. Make sure to black out financial information.
  • Hospital certificate with the child’s footprints on it. Must show the name of the parent who is the subscriber, the subscriber’s spouse or the subscriber’s state-registered domestic partner.
  • Court-ordered parenting plan.
  • National Medical Support Notice.
  • Defense Enrollment Eligibility Reporting System (DEERS) registration.
  • Valid J-2 visa issued by U.S. government.

You can enroll your adopted child in Coverage for Kids. Here is a list of acceptable documents:

  • Birth certificate or adoption certificate.
  • The most recent year’s federal tax return that includes the child(ren) as a dependent  and listed as your child. Make sure to black out your financial information.
  • Certificate or decree of adoption. Must show the name of the parent who is the subscriber, the subscriber’s spouse, or the subscriber’s state-registered domestic partner.
  • Court-ordered parenting plan.
  • National Medical Support Notice.
  • Defense Enrollment Eligibility Reporting System (DEERS) registration.
  • Valid J-2 visa issued by U.S. Government.

You can enroll your stepchild in Coverage for Kids. Here is a list of acceptable documents:

  • Your child’s Birth Certificate as well as government-issued marriage certificate demonstrating relationship to your spouse or partner. Other documents that can be used include a federal tax return within the last 2 years with your spouse listed and proof of joint ownership (mortgage statements, bank statements, credit card statements, lease/rental agreements).
  • The most recent year’s federal tax return that includes the child(ren) as a dependent and listed as your child(make sure to black out your financial information).
  • Court-ordered parenting plan.
  • National Medical Support Notice.
  • Defense Enrollment Eligibility Reporting System (DEERS) registration.
  • Valid J-2 visa issued by U.S. Government.

You can enroll a child you have legal guardianship over in Coverage for Kids. Here is a list of acceptable documents:

  • Legal documentation from state or federal government that shows legal guardianship status.
  • The most recent year’s federal tax return that includes the child(ren) as a dependent and listed as your child. Make sure to black out your financial information.
  • Court-ordered parenting plan.
  • National Medical Support Notice.
  • Defense Enrollment Eligibility Reporting System (DEERS) registration.
  • Valid J-2 visa issued by U.S. Government.

You can enroll your foster child in Coverage for Kids. Here is a list of acceptable documents:

  • Copy of your foster child certification and copy of documentation of regular and substantial support of the child.
  • The most recent year’s federal tax return that includes the child(ren) as a dependent and listed as your child. Make sure to black out your financial information.
  • Court-ordered parenting plan.
  • National Medical Support Notice.
  • Defense Enrollment Eligibility Reporting System (DEERS) registration.
  • Valid J-2 visa issued by U.S. Government.

You can enroll your domestic partner’s child in Coverage for Kids. Here is a list of acceptable documents:

  • Your child’s Birth Certificate as well as government-issued marriage certificate demonstrating relationship to your spouse or partner. Other documents that can be used include a federal tax return within the last 2 years with your spouse listed and proof of joint ownership (mortgage statements, bank statements, credit card statements, lease/rental agreements).
  • Court orders naming the domestic partner as the legal guardian.
  • The most recent year’s federal tax return that includes the child(ren) as a dependent and listed as your child. Make sure to black out your financial information.
  • Court-ordered parenting plan.
  • National Medical Support Notice.
  • Defense Enrollment Eligibility Reporting System (DEERS) registration.
  • Valid J-2 visa issued by U.S. Government.

**If you are enrolling your stepchild or your domestic partner’s child, you will need to fill out additional paperwork. Call SEIU 775 Benefits Group Customer Service for help at 1-877-606-6705 from 8 a.m. to 6 p.m., Monday to Friday.

How do I keep Coverage for Kids?

To keep Coverage For Kids you must:

  • Continue working 120 hours or more per month.
  • Continue paying the full monthly payment (co-premium) for you and your children.

If you lose Coverage for Kids, you will get information about continuing their coverage through COBRA. Coverage for Kids will automatically resume when you start working at least 120 hours per month again. As long as you work at least 80 hours per month, your personal coverage will continue.
