There is always a small risk that your privacy is not fully protected, even though we take steps to keep all information private. The insights gained from the project will be summarized in a final report. The final report will share data as a summary from everyone who completes this survey and will not include your name or any identifying information. As we are collecting your name and your responses together, we will do our best to keep your personal information from being disclosed. Participation in this survey is voluntary. Your participation will not impact any health plan or training you receive from SEIU 775 Benefits Group.
