Create a login on My Plan
Register Today with My Plan
My Plan is a secure, easy-to-use website where you can submit forms, make changes and access information about your plan.
If this is your very first time using My Plan, you will have to register and create a new login.
Follow these simple steps.
Step 1: Go to My Plan.
Click “Register as a New User”.
Step 2: Enter Your Member ID or Your Social Security Number
You will be taken to a new page where you will need to enter either:
- Your member ID number (found on your insurance card), or
- Your Social Security number (SSN).
Then, click “Next”.
Step 3: Enter Your Zip Code and Date of Birth
Next, you will need enter your Zip Code and Date of Birth.
Then, click “Next”.
Step 4: Accept the Terms and Conditions
On this page, you will can read the terms and conditions. Once you have read them, check the box next to “I accept the Terms & Conditions”.
Then, click “Next”.
Step 6: Create a Username and Password, and Select a Security Question
On the same Password page, you will need to select and answer a security question.
The answer to this question is important to remember, because it can be used to reset your password in the future!
Once you have completed your security question, click “Next”.
Step 7: Enter Your Personal Information and Save Acknowledgements
On the final page, you will need to enter your personal information. You will need:
- Your Preferred Phone Number (Home, Mobile or Work).
- Your Email Address.
- Your Preferred Method of Communication.
- Your Preferred Language.
Click “Save Communication”.
